PVMHS CTE Application

This application form must be completed and submitted to the PVMHS CTE Director by Friday,
January 17, 2020. In addition to this application form, the applicant’s current guidance counselor will
submit the criteria used for admission that includes the applicant’s grades, attendance records,
discipline/conduct record and recommendation form/letters.

The undersigned applicant's parent(s)/guardian(s) give permission for representatives of the sending school to release the applicants grades, attendance, and conduct/discipline to the PVMHS CTE Admission Office for the purpose of admission

Information submitted voluntarily by the applicant will not affect the applicant’s admission to the school.
